Behavioral Health Clinician (Bend)
High Lakes Health Care, based in Bend, OR is seeking an LCSW, LPC, or LMFT to join our primary care team as a Behavioral Health Consultant (BHC). Job Description
As part of the primary care treatment team, the BHC will use virtual platforms, information technology, and in-person clinical encounters to identify, triage, and assist in the management of primary care patients in a private practice setting. Our team uses clinical and psychoeducational strategies to support patients with self-management using best-practice and brief evidence based interventions. We provide brief, problem focused, real time assessments/interventions to address a wide range of health and wellness issues using an open-access model that prioritizes timeliness and rapid response, patient satisfaction, and provider well-being.
Your contribution to the team will include:
• Use telehealth and in-person encounters to assist primary health care providers in recognizing and treating mental disorders and psychosocial problems, and behavioral medicine problems.
• Assessment and triage: conduct initial and follow up visits of 30 minutes or less to evaluate level of functioning, determine risk level, assist with diagnosis and behavioral management of HLHC patients.
• Work with primary care provider to refer cases to local or regional mental health specialists as appropriate.
• Evaluate patient care plans with primary care team.
• Teach patients self-care and teach HLHC staff psychological techniques for patient care and prevention.
• Provide assistance in the efforts to grow primary care behavioral health services within the organization.
• Assist in the evaluation of the behavioral health program: review utilization data; establish outcomes to measure quality of life improvements, and cost effectiveness of population-based programming.
• Supervise students and/or interns, and contribute to management of the practicum and internship programs as needed.
Qualifications:
• Master's degree and licensure as LCSW, LPC, or LMFT
• Licensed or license eligible in the state of Oregon
• Flexible, team-oriented, high-energy, and excellent interpersonal communication skills
• Require candidates with strong interest in delivering clinical services.
